Grand Jury Forms

AGE: Persons seventy (70) years of age or older shall be permanently excused upon request. You must provide a completed Grand Jury Service Affidavit and a copy of your ID.

CONVICTED FELON: You may not serve on a Rockdale County Jury if you have not had your civil rights restored. Provide a completed Grand Jury Service Affidavit and a copy of your ID.

DEFERRAL: No one is exempt and/or deferred from jury service because of his/her job, race, color, religion, sex, national origin, sexual orientation, or economic status. In order to be deferred, you must complete a Grand Jury Service Affidavit and submit it to the Jury Clerk at your earliest convenience.

FULL-TIME STUDENT: Full-time students currently enrolled at a college, university, vocational school or other post-secondary school and taking classes or exams on the day of your scheduled service may be excused or deferred. Provide a completed Grand Jury Service Affidavit and attach proof of student status (Current Enrollment Verification Form, Class Schedule, or Letter from Registrar).

MEDICAL: Provide a doctor’s excuse that is specific to your inability to serve as a juror, it can be temporary or permanent condition as indicated by your doctor. No affidavit is required.

MILITARY: Active service members on ordered military duty or spouses of such service members may be excused or deferred. Provide a completed Grand Jury Service Affidavit and attach proof of active military status (Military Orders, Military ID, Letter from Commanding Officer).

NON-CITIZEN: You may not serve on a Rockdale County Jury if you are not a United States citizen. Provide a completed Grand Jury Service Affidavit, and a copy of your Permanent Resident Card.

NON-RESIDENT: You may not serve on a Rockdale County Jury if you do not live in Rockdale County. Provide completed Grand Jury Service Affidavit and attach proof of residence change (drivers license, utility bill, lease agreement, mortgage documents, etc.).

PRIMARY CAREGIVER: Persons who are the primary custodial caregiver of a child six (6) years of age or younger. Provide a completed Grand Jury Service Affidavit.

PRIMARY CARETAKER: Persons who are the primary caretaker for an individual over the age of six who has a medical condition and/or cognitive limitations, and is in need of constant care, may be excused. Provide a completed Grand Jury Service Affidavit and attach a letter from the person’s doctor stating that they need you as a caretaker for their condition.

TEACHER IN A HOME SCHOOL PROGRAM: Persons who are the primary teacher in a home school program, as defined by O.C.G.A. §20-2-690(c). Provide a completed Grand Jury Service Affidavit and attach your current Declaration of Intent.

MENTALLY INCOMPETENT: Any individual who has been judicially determined to be mentally incompetent must provide a letter from your doctor.

PRETRIAL RELEASE PROGRAM: Any individual charged with a felony offense and who is in pretrial release program, a pretrial release and diversion program, or a pretrial intervention and diversion program, as provided for in Article 4 of Chapter 18 of Title 15 or Article 5 of Chapter 8 of Title 42 or pursuant to Uniform Superior Court Rule 27, a similar diversion program from another state, or a similar federal court diversion program for a felony offense must provide a completed Grand Jury Service Affidavit and a copy of your ID.

CONDITIONAL DISCHARGE: Any individual sentenced for a felony offense pursuant to Code Section 16-13-2 who has not completed the terms of his or her sentence must provide a completed Grand Jury Service Affidavit and a copy of your ID.

FIRST OFFENDER: Any individual serving a sentence for a felony offense pursuant to Article 3 of Chapter 8 of Title 42 or serving a first offender sentence for a felony offense pursuant to another state’s law must provide a completed Grand Jury Service Affidavit and a copy of your ID.

ACCOUNTABILITY COURT: Any individual who is participating in a drug court division, mental health court division, veterans court division, a similar court program from another state, or a similar federal court program for a felony offense. Provide a completed Grand Jury Service Affidavit and a copy of your ID.

All Affidavits must be notarized OR signed in the presence of a Deputy Clerk in our Superior Civil office, room 200.
